This case report elucidates a unique strategy for addressing bone loss and first-ray instability complications arising from a failed Keller arthroplasty. A 65-year-old female, experiencing pain and the inability to wear standard footwear after Keller arthroplasty of her left first metatarsophalangeal joint for hallux rigidus five years earlier, presented for care. The first metatarsophalangeal joint of the patient was arthrodest, using the diaphyseal fibula as a structural autograft. Over five years of observation, this previously unknown autograft harvest site successfully treated the patient, leading to a full resolution of their prior symptoms without any complications.

Chronic, non-healing wounds are creating a considerable and escalating healthcare problem in the United States, affecting over 65 million individuals yearly and costing the health care system over $25 billion in direct costs. Chronic wounds, specifically diabetic foot ulcers and venous leg ulcers, are often difficult to treat, with a common occurrence of non-healing, even with the most advanced healing therapies. This research project was formulated to evaluate the therapeutic value and practicality of using the synthetic hybrid-scale fiber matrix in treating complex, chronic non-healing lower-extremity ulcers not responding to advanced medical treatments.
Twenty patients, having a total of 23 wounds (18 diabetic foot ulcers and 5 venous leg ulcers) and treated using the synthetic hybrid-scale fiber matrix, were the subject of a retrospective analysis. Of the ulcers examined in this study, 78% proved recalcitrant to one or more prior advanced wound therapies, thus defining them as difficult-to-heal ulcers with a high potential for failure with subsequent treatment approaches.
The average wound duration for subjects was 16 months, accompanied by the presence of 132 secondary conditions and a record of 65 failed therapeutic interventions. Among the most common treatments are oral or topical antifungal preparations. Although recurrent infections are frequent, the use of systemic oral antifungals is accompanied by concerns over liver toxicity and potential drug interactions, notably in patients on multiple medications. In the area of onychomycosis treatment, several device-based approaches have been established. Their function is either to directly target the fungal infection or to enhance the effectiveness of topical and oral agents. A growing number of people are opting for device-based treatments, including photodynamic therapy, iontophoresis, plasma, microwaves, ultrasound, nail drilling, and lasers, in recent years. Certain treatments, like photodynamic therapy, provide a more immediate therapeutic approach, while others, such as ultrasound and nail drilling, facilitate the absorption of traditional antifungal medications. A systematic review of the literature was undertaken to assess the effectiveness of these device-based treatment approaches. From a collection of 841 initial studies, 26 were identified as addressing the use of device-based therapies in treating onychomycosis. This review delves into these methods, offering a perspective on the current state of clinical research for each. Though promising preliminary results exist with device-based onychomycosis therapies, additional research is needed for a comprehensive assessment of their true impact.

Studies conducted previously revealed that second-stage juveniles (J2) of Meloidogyne species are drawn to some benzenoid aromatic compounds. Fluopyram-treated tubes attracted a substantially higher concentration of M. javanica and M. marylandi J2 larvae, reaching levels 44 to 63 times greater than that observed in fluensulfone-treated tubes.
